Set up the timer before leaving the menu
Choose Options, then Game Options, then Show Time Played. GameRant documents a Detailed setting showing total and current-section time in the top-right corner. Use the tracked total to judge your run. Pause before looking up the next step or taking a break; do not assume that holding a map or standing at a puzzle has the same effect as the pause menu.
Choose a replay you can finish consistently
A familiar route is easier to plan than a blind first visit. The trophy does not require a higher difficulty, so Story Mode with Hinted puzzles is a reasonable setup. New Game+ is useful but not required. Its Axe is a pickup after a completed game; the Pistol-3000 needs a prior Sci-fi ending. Neither should be assumed to be in your starting inventory.

Fleeting Visit does not impose the separate Reject The Help rule against phone saves after meeting Richard. Keeping useful saves can protect a speedrun attempt, but conflicts with that other trophy. Decide which objective matters before using a phone.
A fast run focused on combat may also change your ending. If Justice is another goal, follow the Discoveries-versus-Kills guidance. Do not mix an all-endings plan into a speedrun halfway through without checking its required items and save points.
Check completion, not just the last checkpoint
Finish the ending sequence and check the recorded playtime and achievement notification. Reaching the final area before four hours is not the completion requirement.
